You can try to clean it with alcohol on the contacts if it's physically dirty.  If you've cracked or broken your action replay, there are no user serviceable parts inside.  In other words, if you've physically broken it, you'll need to buy a new one.

According to the codejunkies web site, yes, it comes with everything you need.  If you buy it used, check with your retailer to be sure it comes with everything you need.
