you dont but if u use the fist and point it at the ashes then it is a wepon and then use it on the bear bottle that is standing up

 use the knife... after that hop in, get the pipe, "its by the ladder", then get out of the sewers...

go to the alley, there will be somebody throwing trash out a window, cut is open with the knife, you will get "food, and tools" go back  into the sewer and go into the hole.  put the burger by the hole "place burger under you" crock will come out. use the pipe... then the rest is to your-self
